Credit Authorization & Disclosures

By submitting your application through Forge Capital & Leasing (a DBA of SLDavid LLC), you authorize us and our funding partners to obtain credit and business information as necessary to evaluate your financing request.

1. Authorization

You authorize Forge Capital & Leasing, its agents, and associated funding partners to:

• Obtain consumer and business credit reports from credit bureaus or third parties.
• Verify employment, bank accounts, trade references, or other credit references.
• Share your information with funding sources solely for the purpose of credit evaluation.

2. Certification

You certify that:

• The information provided is true and complete.
• You are authorized to submit this application on behalf of the business entity.
• You understand that false statements may result in denial or termination of funding consideration.

3. Disclosure

Credit inquiries made in connection with commercial financing are not consumer credit inquiries and may not affect your personal FICO score in the same manner as consumer credit inquiries.

4. Right to Withdraw

You may withdraw your authorization by contacting us in writing prior to submission for review. Once your application has been submitted to a lender, this authorization remains valid for up to 90 days.
